Terms and Conditions:
These terms and conditions constitute the agreement (“Agreement”) between you and Cardio Prognostics, Inc. (“CPI”) with respect to the services “of acquiring your Electrocardiogram (“ECG”) and conducting an analysis of that ECG for the purposes of evaluatory examination for behavioral characteristics believed to be signs that element of Alzheimer’s disease may be developing” (the “Service”) to you (“the Parties”). Your use of the Service constitutes your agreement to be bound by the terms of this Agreement. In this Agreement, the words “you” and “yours” mean each person who applied for the Service, and the terms “us” and “we” mean CPI. This Agreement is in addition to other agreements between the Parties.
Online Registration, Scheduling, Correspondence, and Offline ECG Evaluatory Service
The Online Registration is an alternative to receiving mailed, emailed, paper account setup requests or person to person phone call account setup. By applying for the Service, you authorize CPI to correspond with you through electronic services like email and text. You may designate the correspondence methodology to be all or any of those available in the settings of your account registration.
The Service will send an email reminder to the email address you provide through Online Registration at least 24 hours prior to any scheduled appointment. It is your responsibility to keep your email information current. Email addresses may be updated at any time by selecting Settings in your Online account.
You agree not to give or make available your online password to any unauthorized individuals. You agree that it is your responsibility to examine your online account settings and to contact CPI as soon as possible to report any irregularities or disputes. Records of evaluation and ECGs and any legal notices about your account will remain available online for at least 18 months. You may also print or download your ECGs and results to your computer, without charge.
It is your responsibility to provide and maintain the proper hardware and software (computer and/or mobile devices) that will enable you to view account information online, which is the same hardware and software as is used for scheduling an submitting your ECGs and utilization of the Service. Downloaded information will be provided in Adobe Portable Document Format (PDF). To access and retain a PDF file, you will need the free Adobe Acrobat Reader (version 4.05 or higher) or equivalent PDF reader.
The terms of this Agreement, be altered or amended by CPI from time to time, with or without notice to Online registrants. You agree that electronic delivery or posting of disclosures, notices, modifications and other communications about the Service provide by CPI has the same effect as mailing them to you. CPI cannot guarantee receipt of notices. Any use of the Service after CPI institutes and/or posts a change will constitute your agreement to such change(s). Further, CPI may from time to time revise or update its programs, and/or service and informational material(s), which may render all prior programs, and/or service and informational material, obsolete. Consequently, CPI reserves the right to terminate its Service as to all such prior offerings of the Service, and/or related material, and to limit access to the CPI’s more recent revisions and updates of the Service, without obligation of prior notification to registrants and/or users of the Service.
CPI may terminate use of the Service by any individual at any time. Neither termination nor discontinuance shall affect your liability or obligation under this Agreement.
Upon our request, you agree to sign a non-electronic version of this Agreement. At your request, we will provide you with a paper copy of any of the regulatory disclosures or notices about the Service posted electronically without charge by contacting us though our web services.
The test results are NOT a diagnosis. The test results simply provide indicators of your vulnerability of the disease so that your physician can diagnose and choose early treatment.
